Hi,
you have obviously been in very good shape in the past and know your stuff about traing,diet etc. My advice would be to start off slowly, do not go at it like a bull at a gate. Do some c.v., moderate, and some weights, again moderate. Build the intensity up as you feel yourself getting fitter and stronger. Because you have been down this route before your body has, what I believe, is "muscle memory". I have had lay offs, and come back more quickly than if I had not trained at all. Your state of mind needs to be good too, what I mean by that is you want to have to want to do it. The mind set is such a strong thing. I visualise my workouts before I do them, how its going to feel, what effort I am going to have to put in, how the muscle will feel when its full of blood. If you have your head right for this then you WILL do it.
If you need specifics then ask, but I suspect you know what to do, you just need reassurance.
